I wouldn't say it is nastier than Last House On The Left, but it's just as fucked up.
Once you get past the typical Italian styled English dubbing, the movie plays itself straight with NO comic relief.
One very cool trivia, see if you recognize the main "harmonica/train" theme by Ennio Morricone (which also comes up on the DVD's main menu page)- he repeated it (or paid homage to it) in a classic American movie in the 80's.
I know when I watched The Strange Vice Of Mrs. Wardh (1970) - I immediately recognized its main melancholic theme music from Kill Bill Vol. 2.
